Requesting a Consultation

A Research Ethics consultation can be initiated (1) electronically or by (2) contacting a consultant:

1) Ethics Support Request Form – Please fill out the form as completely as possible.

Purpose of Request: This form may be used to request the variety of services offered by Ethics Support including:

Follow-up Requested: Please indicate if you have a preference as to how you would like us to follow up with you.

Study Information: If your request involves a study protocol, please provide a few details to better assist us.

Nature of Request/Inquiry: Please provide as much detail as possible to best assist us in addressing your request.

Other Parties Involved: It is important for us to know who may be stakeholders regarding your request. We recognize that an ethics consultation may involve sensitive information. Please indicate whether or not you would like us to contact anyone mentioned above and if not, please provide rationale.

Additional Comments: Please include any other information you feel is pertinent for us to better assist you.

The form contains a ‘Submit’ button for your convenience.

2) A consultant may be contacted by phone at 409-747-1230 (after-hours and on the weekends and holidays a recorded message supplies a pager number to reach the on-call consultant). Or, you may call the director at (409) 772-1208. If you are on campus, just dial: 2-1208.
